OEBB Open Enrollment

District staff will be in the buildings on August 27th and 28th to answer questions and help with enrollment, or you can call the district office at 503-325-6441

OEBB Benefits Summary

Medical/Pharmacy Plan: All staff members must enroll in MODA Plan 5The district provides a Group HRA (Health Reimbursement Arrangement) in addition to Plan 5 to help lower your out-of-pocket costs.

Dental/Vision Plans: Employees can enroll in the Dental and/or Vision plan of their choice.

Wellness Resources: Employees have access to dozens of wellness programs for free.

  • All staff members must enroll in MODA Plan 5

    Premium Rate: The amount paid each month for health insurance coverage. Think of it like a subscription fee to keep your insurance active.

    • Classified Part-time Employee: The District pays 100% of your premium only. Optional dependent costs are your responsibility and will be deducted from your paycheck.

  • Employees may choose to enroll in Dental and/or Vision Plans

    Dental/Vision Premium Coverage:

    • Classified Part-time employees: The District pays up to $95.00 per month* for your dental and vision insurance premiums. ​​​​​​​Download Classified PT Rate Sheet

    ​​​​​​​*If your plan(s) cost more than what the district pays, the extra amount will come out of your paycheck through payroll deduction.

  • Diversified Benefits Services Group HRA

    • Create your account: Visit the Diversified Benefits website  and click “User Login” 
      • Click “Create New Account”
      • Enter Employer PIN: AstoriaSD  and click the red arrow
      • Enter your information and click submit

     

    • EOB’s -When you receive medical services, an Explanation of Benefits (EOB) from MODA is created and is automatically sent to the HRA.

     

    • Reimbursement: The HRA will reimburse eligible expenses toward your deductible, coinsurance, and copays (including prescriptions).  Payments are made directly to you by direct deposit.

    Deductible: The amount you must pay for covered medical services before your insurance starts sharing the cost.

    Copays: A fixed amount you pay for a specific service, like a doctor's visit or a prescription. Copays count towards your out-of-pocket maximum.

    Out-of-Pocket Maximum: The most you’ll have to pay for covered healthcare in a plan year for your deductible, prescriptions, coinsurance & copayments

    • Single Deductible: $2,400 total
      • First $700: You pay
      • Next $1,700: HRA pays 75% ($1,275) / You pay 25% ($425)
      • Out of Pocket Maximum: $3,750
    • Family Deductible: $4,800 total
      • First $1,400: You pay
      • Next $3,400: HRA pays 75% ($2,550) / You pay 25% ($850)
      • Out of Pocket Maximum: $7,500
  • How to Enroll in Your Benefits – Step by Step

    Step 1:
    Log in to MyOEBB.org during the open enrollment period of August 15 through September 15, 2025

    Step 2:
    Sign in with your username and password.

    • Forgot your username or password? Click the “Forgot User Name or Password?” link below the sign-in button.

    • First time using MyOEBB? Click “Create your MyOEBB account” and follow the prompts.

    Step 3:
    On the landing page, choose “Enroll, Change, Opt Out, or Waive Benefits.”

    Step 4:

    Follow the on-screen prompts and answer all demographic questions.

    • Enroll in MODA Medical Plan 5 (the only option per contracts).
    • Choose any Dental and/or Vision plan if desired.
    • If you have other insurance coverage you can choose to Opt Out or Waive Benefits

     

    Step 5:

    When you reach the Benefit Statement page, review your information and selections.

    • If everything looks correct, scroll to the bottom, check “I have reviewed and agree,” and click “I agree.” Important: Your benefit selections won’t be saved unless you complete this step!

    Step 6:
    Watch for a confirmation email from OEBB. Your new benefits will take effect on October 1, 2025.

    Step 7:

    Download the HRA Direct Deposit Form here, fill it out, and return it to the district office. If you already have a direct deposit set up through payroll, check the box “Use banking information on file with the District.”

  • MODA Health is partnering with Garner

    August 15 – Moda members can begin registering with Garner

    October 1 – Reimbursements for Garner-approved visits begin

     

    Garner is an optional additional benefit that helps you find the top doctors in your area. 

    When you visit a Garner-approved provider, you can be reimbursed for eligible costs, including deductibles, copays, and coinsurance, Labs, prescriptions, and X-rays 

    Annual Reimbursement Limits:

    • $700 for individual coverage

    • $1,400 for family coverage

    Important:

    • You are not required to use Garner—your Moda network stays the same.

    • To be eligible for reimbursement, you must register with Garner before your visit and see a Garner-approved provider.

    How to Get Started:

    1. Review the Garner Guide to learn how to enroll, find providers, and get reimbursed.

    2. Register at Get Garner or through your Moda Health dashboard.

    3. If you have a PCP 360 with Moda, check in Garner to see if they are approved.

  • Canopy Employee Assistance Program (EAP)

    Canopy is available to ALL Astoria School District Employees and their families and provides free and confidential services, including counseling, legal, and financial help. To register with Canopy, use “OEBB” as your organization name.

  • Basic Life Insurance and Optional Life Plans

    The District pays for a $20,000 basic life insurance policy for all employees. You can add extra life insurance or disability insurance for yourself or family members through our vendor, The Standard:

  • Have Other Health Insurance?

    If you have other health insurance, you can choose to opt out of the district’s medical, dental, and vision coverage. Instead, the district will deposit a set amount (per your contract) into an HRA VEBA account for you.

    What is HRA VEBA?
    A tax-free account you can use to pay eligible medical, dental, and vision expenses—even in retirement. Funds roll over from year to year and never expire.

    Important:

    • You must waive all district medical, dental, and vision coverage.

    • Medicare and Tricare recipients are not eligible.

    • Proof of other insurance is required.

  • Oregon Paid Family Medical Leave

    Astoria School District has partnered with The Standard for Paid Family Medical Leave

  • PenServe 403(b) 

    Employees interested in contributing more to their retirement can do so voluntarily to a 403(b) TSA plan or a 457(b) deferred compensation plan. Astoria School District has partnered with PenServe.
