CMH will be offering walk-in sports physicals for students.

When:  August 7 & 8
9 a.m. – 12 p.m. & 1 p.m. – 4 p.m.
(Closed for lunch from 12–1)
Where: Astor Elementary School, 3550 Franklin Ave.
Cost: $30 (For financial assistance, contact your school on Aug 4)
No appointment needed – first come, first served

All students planning to participate in sports must have a current physical.
